IVARA Pty Limited (ABN 79 696 057 075) (“IVARA”, “we”, “us”) is bound by the Privacy Act 1988 (Cth), including the Australian Privacy Principles, and the Health Records and Information Privacy Act 2002 (NSW). This policy explains how we manage personal and health information.
What we collect
Depending on how you deal with us, we may collect:
- names, contact details and employment details of workers, employers and referrers;
- claim information, such as claim numbers, dates of injury and insurer details;
- health information, including medical certificates, reports and information about injury, capacity and treatment;
- information about education, skills and work history, where relevant to returning to work;
- information you send through this website, including referral and contact forms.
How we collect it
We collect information directly from you wherever possible. We also receive information from insurers, employers, treating practitioners and other parties involved in a workers compensation claim, as permitted by law and the workers compensation legislation.
Why we use it
We use personal and health information to:
- assess and deliver workplace rehabilitation services;
- communicate with the worker, employer, insurer and treating practitioners;
- prepare plans and reports required in the NSW workers compensation scheme;
- meet our legal and regulatory obligations, including to SIRA;
- respond to enquiries and improve our services.
Who we share it with
We share information only as needed to deliver services and as permitted by law, typically with the insurer, the employer (limited to what is needed to support the return to work), treating practitioners, and regulators such as SIRA. We do not sell personal information.
How we keep it secure
We store information in secure systems with access limited to staff who need it. Information sent through this website is transmitted over an encrypted connection. We retain health records for the periods required by law and then securely destroy them.
